This website
This website does not have contact forms, accounts or online booking, and does not ask you to submit health information. When you visit:
Analytics – we use Google Analytics to count visits and see which pages are useful. It uses cookies and collects information such as your approximate location, device and the pages you view. It does not tell us who you are.
YouTube videos – videos are shown with a preview image; when you play a video, YouTube (Google) may set cookies and collect information under its own privacy policy.
Maps – the contact page shows an embedded Google Map, which Google may use to set cookies.
Our web host keeps standard server logs (such as IP address and pages requested) for security.
You can block or delete cookies in your browser settings; the site will still work.
When you contact us or become a patient
If you phone, email or attend the practice, we collect the information needed to care for you and to claim from your medical aid: your contact details, ID number, medical aid details, medical history, test results and clinical notes. Health information is special personal information and is processed only for your treatment and care, for claims and authorisation with your medical aid, and where the law requires it.
We share it only with those involved in your care (for example the hospital, referring and treating doctors, laboratories and radiology), with your medical aid, and where required by law. Records are kept securely for at least the periods required by law and the HPCSA.
Your rights
Under POPIA you may ask to see the personal information we hold about you, ask us to correct or delete it where appropriate, and object to its processing. To do so, contact the practice at the details above. You may also lodge a complaint with the Information Regulator (inforegulator.org.za).
Access to information (PAIA)
Requests for access to records under PAIA may be made to the practice at the details above. The practice's PAIA manual is available on request.
Changes
We may update this notice from time to time. The date at the top shows when it was last changed.
